Tsunade and Naruto are distantly related. She is the descendant of the Senju and Uzumaki Clan, her grandfather is Hashirama Senju and her grandmother is Mito Uzumaki. And we know that the Senju Clan descended from the Sage of the Six Paths, thus meaning so do the Uzumaki Clan, as they are distant blood relatives of the Senju. Thus, proving Tsunade is related to Naruto, but not as distant as you may think.
Well, I know they are related, but does Naruto (not us) ever learn during the show that Tsunade is a distant relative?

Sort of, when Kushina told him the story of the Uzumaki and Senju relationship. But they never explored it beyond that. He already knew about Tsunade's relationship to the First Hokage since Part I. I'm sure Tsunade knew all along since she also knew about Naruto being Kushina's (and Minato's) child.
