Kurama Mode is Naruto's name for the shroud he has when entering Tailed Beast Mode. Meaning Minato used Kurama Mode when he manifested Yin-Kurama, though it didn't look all that different from his basic Nine-Tails Chakra Mode, with just the slit pupils as an addition.
Naruto's Mode in The Last was not Nine-Tails Chakra Mode but Kurama Mode (combined with Sage Mode as well), since he had to manifest Kurama outside of his body.
Kurama Mode and Tailed Beast mode are the same thing, the Jinchuriki have access to transform into full beast Kurama replica and have slit pupil and can use Tailed Beast Ball. Minato was in chakra mode but was also able to access that mode during the war he had slit pupil and transformed into full Kurama, see this image. Naruto used Kurama mode in the Last, although it is called Kurama chakra mode it is still Kurama mode as he was able to summon Kurama.

Kurama Mode is the shroud around the jinchūriki's body. Tailed Beast Mode is the full transformation into the tailed beast, in this case, Kurama.

It's actually called "Kurama Chakra Mode" (九喇嘛チャクラモード) in the official stuff.
